Solliva patient app — Privacy Policy Section 10

10.1 Overview. Depending on where you live, you may have some or all of the following rights with respect to personal information Solliva holds outside its HIPAA service provider role: to know or access it; to receive a portable copy; to correct it; to delete it; to opt out of sale, sharing, targeted advertising, and certain profiling; to limit the use of sensitive personal information; and to appeal a refusal. Information Solliva handles as PHI under HIPAA is generally exempt from these state laws; Section 11 explains the rights that apply to it instead. 10.2 How to exercise rights. Submit requests through the App’s privacy settings or by email to contact@solliva.com. Solliva will verify your identity using account information and reasonable additional confirmation where the request is sensitive, will respond within the time your state’s law requires (generally forty-five (45) days, extendable once with notice), and will not discriminate against you for exercising a right. An authorized agent may submit a request on your behalf with proof of authority; Solliva may still require you to verify your identity directly. If Solliva declines a request, the response will explain why and how to appeal, and appeals will be decided within the period your state’s law requires; if your appeal is denied, you may contact your state attorney general.

Solliva Physician — Privacy Policy Section 7

7.1 State privacy laws. Depending on your state of residence, you may have rights to know, access, correct, delete, or receive a copy of personal information Solliva holds about you, and to appeal a refusal. These rights apply to your information as an Authorized User; they are subject to exceptions, including where retention is required by law (audit logs, for example, cannot be deleted on request) and where the information is your organization’s Confidential Information or a patient’s PHI. California residents have the rights provided by the California Consumer Privacy Act as amended, which applies to workforce and business-to-business personal information. 7.3 How to exercise rights. Submit requests to contact@solliva.com. Solliva will verify your identity, coordinate with your organization where it supplied or controls the information, respond within the period your state’s law requires (generally forty-five (45) days, extendable once with notice), and explain any refusal and how to appeal; if your appeal is denied, you may contact your state attorney general. An authorized agent may submit a request on your behalf with proof of authority. Solliva will not retaliate against you for exercising a privacy right; your organization’s independent decisions are outside Solliva’s control. 7.4 Corrections through your organization. Role, employment, and credentialing information is controlled by your organization; corrections to it should be directed there, and Solliva will reflect the correction once made.
